We are keen to share you medical notes, results, immunisations and medication lists with you. This can be very helpful when you visit a specialist, travel out of the area, wish to view past test results or want to track your progress as you work on improving your health. Please let us know as soon as you move or change phone numbers. We often need to get in touch with you between appointments to remind you about recalls or follow up test results or hospital requests.
